Reopen a Self-Disclosure

If a Self-Disclosure was completed incorrectly or inaccurate information was provided in error, follow the instructions below to have the Self-Disclosure corrected and resubmitted. This action can only be performed by Hiring Entities (Verifiers) in states with unprofessional conduct or employment history review forms that include self-disclosures.


Important Note: Contact Support to update the employee’s first name, last name, date of birth, or last four digits of their SSN. These changes must be completed before the employee restarts the verification.


 

Reopen Self-Disclosure


 • Login and visit the Completed EV Requests page.

 • Locate the Self-Disclosure needing to be reopened.

 • Click the ellipsis and then Re-Open Self-Disclosure.

 

 


 • Enter the reason for reopening and click Confirm.




The form has been moved to ‘Pending EV Requests’. Once complete, it will move to ‘Completed EV Requests’.



The Completed Form

The completed Self-Disclosure will now include:

    • The new Self-Disclosure.

    • An audit log of who reopened the form, when the action took place and the reason for reopening.

    • The Self-Disclosure initially completed.
